JOB TITLE: Community Health Nurse, Hours TBD, one year term position with possibility of renewal
POST: Dakota Wicozani Tipi Health Centre
SALARY: Salary & hours to be determined based on experience and availability
JOB DESCRIPTION: The Community Health Nurse is part of the Health Promotion and Prevention team at Dakota Wicozani Tipi Health Centre. The Community Health Nurse will provide culturally appropriate awareness and support to community members. The Community Health Nurse collaboratively strives to improve the health of all people and reduce inequities among populations by addressing determinants of health and promoting equitable health outcomes. The Community Health Nurse will facilitate group sessions, workshops and screenings for chronic diseases. The Community Health Nurse will assist to manage communicable diseases and chronic diseases in the community.
Qualifications:
- RN or LPN with current Active Practice Registration and in good standing with their respective Colleges
- CPR and First Aid Certification
- Knowledge in the current theories and best practice guidelines related to Diabetes
- Diabetes Educator Certificate is an asset
- Experience with case management and contact tracing of STBBIs an asset
- Demonstrated ability to integrate and apply both behaviour change and harm reduction concepts
- Demonstrated commitment and knowledge of community-based health care
- Must possess sound clinical judgement
- Ability to work independently and with a team in a variety of settings
- Strong leadership, critical thinking, decision making and problem-solving skills
- Ability to develop partnerships with communities and organizations
- Effective verbal and listening communications skills
- Knowledge of First Nations culture
- Must have valid driver’s license and reliable vehicle
- Commitment to professional development
- Current Criminal Record Check, Vulnerable Persons check and Child Abuse Registry Check must be available upon request of successful candidate
